CAPITAL CONTRIBUTIONS AND SHARES
As a legally binding addendum and amendment to our partnership agreement dated [Date of agreement] we, [Name of first partner] and [Name of second partner] submit the following:
[Name of first partner] shall contribute [Description of contribution] representing [Cash value] the following percentage of the value: [Percentage of value of first partner's contribution].
